Established as a public charity in 1997 as the result of the merger between Phoenixville Hospital with the University of Pennsylvania Health System.

In August of 2004, the Phoenixville Hospital was sold to Community Health Systems of Brentwood, Tennessee, a national for-profit health system. As a result of this sale, the Foundation's status changed from that of a supporting public charity to the non-profit Phoenixville Hospital to a private foundation with no organizational tie to the now for-profit Phoenixville Hospital.

The mission of Phoenixville Community Health Foundation is to improve the health and quality of life in the community we serve. The Foundation has taken a broad definition of health. This definition encompasses prevention and education and requires that the Foundation, through its grantmaking and community efforts, serve to improve the quality of life of the community. The Foundation seeks to become the

"CHAMPIONS OF THE COMMUNITY".

The Foundation has adopted the same geographical areas served by Phoenixville Hospital. This area is defined as follows:
